If you communicate with us by telephone, we may ask you to provide us with contact information, such as your name, date of birth, mailing address, telephone number, e-mail address, billing information and other payment information.
In addition, you may be asked to provide any other personal information necessary for your care by one of our hearing healthcare professionals, such as your health insurance number, medical test results, medical history, lifestyle habits, etc;
Visit our offices
If you visit our offices, you may be asked to register as a visitor and provide your name, date of birth, postal address, e-mail address, telephone number, as well as the time and date of your arrival and your next appointments, if any.
In addition, you may be asked to provide any other Personal Information necessary for your care by one of our hearing healthcare professionals, such as your health insurance number, medical test results, medical history, lifestyle habits, etc;
IMPORTANT: If you provide us with Personal Information about other individuals, you represent that you have the authority to do so and, where applicable, that you have obtained the necessary consent, and you acknowledge that such information may be used in accordance with this Policy.
2. WHY IS YOUR INFORMATION COLLECTED AND HOW IS IT USED?
Laflamme et Associés, Audioprothésistes Inc. collects and uses your personal information for :
a) to provide you with services and information concerning the services offered;
b) manage your file, communicate with you and maintain your customer account;
c) process payments that you may be required to make, in particular the payment of your invoices;
d) to send you information concerning the various services offered or information you have requested, or to respond to your requests and questions;
e) facilitate communications, in particular by collecting information on your device data and your telephone number;
f) communicate with you (by telephone, alert, e-mail, mail or otherwise) and provide you with additional information about our services, surveys and customer service;

Laflamme et Associés, Audioprothésistes Inc. strives to limit the collection of Personal Information to that which is strictly necessary to fulfill the purposes for which it is collected. We will not disclose or use your Personal Information for any other purpose, but note the following clarifications and exceptions:
Law Enforcement: We may share your Personal Information to respond to law enforcement requests, court orders or other legal process or if we believe that such disclosure is necessary to investigate, prevent or respond to illegal activities, fraud, physical threats or as required by any other applicable law or regulation and to the extent necessary to protect the property, interests and rights of Laflamme et Associés, Audioprothésistes Inc. .
Archival Service: We may share your Personal Information with a company engaged in the business of preserving documents for their general informational value.
Our employees (Authorized Personnel) and authorized service providers.
Authorized Personnel may have access to your Personal Information in order to assist us in providing services to you. Authorized Personnel access and use such Personal Information in accordance with our instructions, only on a need-to-know basis, and under strict confidentiality and security obligations. Please be assured that our Authorized Personnel manage such information with the utmost discretion and rigor, and comply with this Policy and applicable legal and regulatory requirements.
Business Transaction: Laflamme et Associés, Audioprothésistes Inc. may be involved in (1) the sale or lease of all or substantially all of its business and/or assets, (2) the modification of its legal structure by merger or otherwise, (3) a transaction to obtain a loan or other form of financing, and (4) any other similar transaction (individually and collectively the "Transaction"). As part of such Transaction, we may transfer or disclose your Personal Information to the counterparty organization, actual or potential, but we will require such organization to agree to protect the confidentiality of your Personal Information in a manner consistent with this Policy.

4. ACCESS, RECTIFICATION AND UPDATING OF PERSONAL INFORMATION
You have the right to access and obtain copies of your Personal Information. You also have the right to update, rectify and correct any Personal Information that you believe is inaccurate or incomplete. To do so, please submit a written request to this effect by writing to the Privacy Officer at the address indicated in clause 14, who will respond to you in writing within 30 days of the date of receipt of any such request.
5. YOUR PERSONAL INFORMATION AND THIRD PARTIES
Our Site may reference, integrate or link to third-party sites. These third party sites ("Third Party Sites"), whose links are provided solely for your convenience, are not under our control. These Third Party Sites are governed by their own privacy policies and may collect your Personal Information. The Third Party Sites are not developed, sold or designed by Laflamme et Associés, Audioprothésistes Inc. Accordingly, Laflamme et Associés, Audioprothésistes Inc. does not warrant and is not responsible for the collection, use, retention or disclosure of your Personal Information by Third Party Sites. This Policy does not apply to the practices of third parties and does not assume any responsibility for your use of such third party Sites or your access to third party Sites through our Site. At no time will Laflamme et Associés, Audioprothésistes Inc. sell or trade your Personal Information for a fee.
6. TRANSFER OF PERSONAL INFORMATION ABROAD
Please note that your Personal Information is currently hosted in Quebec.
In the future, we may engage third-party service providers to process, manage and host Personal Information on our behalf, which may be located outside Canada. As a result, some of your Personal Information may be transferred to another province or country and may be subject to the laws of that province or country.
However, we take care to protect the Personal Information in our custody, including Personal Information entrusted to a service provider, whether located in Canada or abroad. Our policies and practices regarding the protection of Personal Information require that a written undertaking be signed by the service provider to respect its obligation to preserve the confidentiality and security of the Personal Information entrusted to it. This includes implementing robust and effective security measures, as well as prohibiting the disclosure of your Personal Information to third parties in accordance with the applicable privacy legislation in the province of Quebec.
7. PROTECTION OF PERSONAL INFORMATION
We take appropriate measures to ensure the confidentiality of Personal Information and to protect such Personal Information against loss, as well as unauthorized access, disclosure, copying, use or modification, taking into account, in particular, the sensitivity of the information and the purposes for which it is to be used.
Laflamme et Associés, Audioprothésistes Inc. uses commercially reasonable and appropriate physical, electronic and managerial procedures and generally accepted industry standards to safeguard and secure the Personal Information we collect.
In addition, the Personal Information we hold is therefore only accessible to persons who are entitled to know about it and who consult it only when necessary for the performance of their duties.

8. MINORS
Our Site is intended for use by persons 18 years of age and older. Laflamme & Associées, Audioprothésistes Inc. does not knowingly collect Personal Information from children under the age of 18.
If a minor under the age of 14 wishes to use our Site, we will not collect Personal Information from that minor without the consent of the parent or guardian. If we become aware that we have unintentionally collected Personal Information from persons under the age of 14 without the consent of their parent or guardian, we will ensure that such information is deleted from our records as soon as possible. 9. RETENTION PERIOD
With the exception of non-identifiable and anonymized information that Laflamme et Associés, Audioprothésistes Inc. may retain in perpetuity and in any reasonable location, Laflamme et Associés, Audioprothésistes Inc. will host and retain your Personal Information for as long as necessary to fulfill the purposes set out in this Policy and to ensure compliance with our legal and regulatory obligations. You may at any time withdraw or change your consent to our use of your Personal Information, or request that we permanently delete it, subject to conditions imposed by law.

11. COOKIES AND OTHER TRACKING TECHNOLOGIES ON OUR SITE, THROUGH OUR SERVICES AND IN E-MAIL COMMUNICATIONS.

Each of these provisions also applies to any dispute that may arise in connection with this Policy and the collection, use, retention and disclosure of your Personal Information, and shall have the same force and effect as if they had been reproduced verbatim in this Policy.
